• ANNEXE 1 - MANDATORY CONDITIONS

   

  1        There shall be no sale or supply of alcohol when there is no Designated Premises Supervisor (DPS) in respect of this premises licence or at a time when the said DPS does not hold a personal licence or when his/her licence is suspended. 

   

  2        Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
   

  3        The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
   

  4        The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
   

   

• ANNEXE 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE

   

  1.  A CCTV system shall be installed at the premises.

   

  2.  A CCTV screen shall be installed at the entrance to the premises to ensure that the public are aware that the system is in operation.

   

  3.  A CCTV screen shall be installed in the back office of the premises, so that the screen can be monitored from time to time by the staff.

   

  4.  Digital CCTV recordings shall be kept for 31 days.

   

  5.  A notice shall be displayed at the entrance to the premises advising that CCTV is in operation.

   

  6.  A notice shall be displayed at the entrance to the premises advising that all shoplifters will be prosecuted.

   

  7.  A suitable fire risk assessment in accordance with the Fire Precautions (Workplace) Regulations shall be conducted at the premises and the necessary control measures shall be implemented.

   

  8.  Fire safety equipment shall be maintained in good working order.

   

  9.  Fire exit signs shall be displayed at the rear and front entrance/exits of the premises.

   

  10.  Emergency lighting shall be installed at the premises.

   

  11.  All electrical appliances shall be tested regularly.

   

  12.  A proof of age policy shall be operated, whereby anyone appearing to be under the age of 21 shall be asked for proof of age when purchasing alcohol.

   

  13.  All staff shall be trained on the proof of age policy.

   

  14.  A notice shall be displayed on the door to the premises informing customers that it is an offence to purchase alcohol for someone who is under age.

   

  15.  The sale by retail of alcohol shall be confined to the area (s) as outlined in RED on the attached plan (s).
